
The Family Journey works with community partners across St. Joseph and Elkhart counties to offer many different services to our participants. Our goal is to connect families with the resources and care they need during their perinatal (pregnancy, postpartum and new parent) experience.
You are not alone on this journey—there are people across this community who are here to support you every step of the way. Participation in this program is free of charge.
The Family Journey helps connect you and your infant with medical care by working with Federally Qualified Health Centers and health systems in St. Joseph and Elkhart counties.
We also work with a nurse practitioner who provides care to Family Journey participants through our home visiting program.
Clinical services include:
The Family Journey participants receive care coordination from our staff and community partners. This coordination includes providing support to participants as well as connecting them to community resources.
Participants will receive monthly visits or check ins from a certified community health worker who will provide guidance through every step of their family journey.

The Family Journey provides participants with both one-on-one and group educational opportunities that help prepare and empower families to welcome a new child into their home.

It is normal to feel a range of emotions during your or your partner’s pregnancy and as you welcome your new child into the world. That’s why The Family Journey participants receive support and care from area mental health professionals through a range of services.
