The Family Journey: A Healthy Start Initiative is a free* program that provides support to pregnant women and families. Through coordinated care, case management, home visits and education, The Family Journey team is here to support pregnant women, partners and infants to increase access to prenatal care and remove barriers to services.
It is our goal that every family has what they need to have a healthy pregnancy and ensure their children thrive.
*Some clinical services may bill insurance.
We will mobilize the community to support all pregnant and postpartum women and families regardless of race, socio-economic status and geography. Additionally, we seek to understand the barriers faced by pregnant women and new parents in our communities, find solutions and advocate for them so that we can empower pregnant women and families to advocate for themselves.

The Family Journey works with pregnant women and families in St. Joseph, IN and Elkhart, IN counties with children up to 18-months of age. Multiple services are available in English and Spanish.
If you reside outside of these two counties, please feel free to still fill out our referral form linked at the top of the website under “Enroll Now” and we will get you connected to the support and services you need.
The Family Journey provides services to ensure people have healthy pregnancies and that they, along with their partners, are empowered to raise healthy children.
Services provided by The Family Journey include:
We partner with several community organizations who assist in providing mental health support services, healthcare, transportation and more for our new and expecting parents.
